Pregnancy care management
These services are available to pregnant people who qualify for Medicaid. We work with you to help you have a healthy pregnancy. Our services include helping you:
- Identify health risks
- Get to your appointments
- Keep track of your appointments and medications
- Schedule health care visits after you have had your baby
- Get other help you may need

Counseling services

Postpartum and newborn home visiting
We offer new parents a one-time home visit after they leave the hospital. During our visit, our nurse will check on you and your baby. We can

- Answer questions about infant care
- Help you schedule medical appointments, and
- Make sure your home is safe for baby
